目的 研究慢性阻塞性肺病 (COPD)患者血白细胞介素 - 8(IL - 8)、肿瘤坏死因子α(TNFα)和内皮素 - 1(ET1 )含量与肺功能关系。 方法 采用特异性放射免疫技术测定 COPD急性期 2 6例、缓解期 2 0例和健康对照组2 0例血 IL - 8、TNFα和 ET1 含量 ,并测定肺功能。 结果 COPD急性期 IL - 8含量 (2 48.2 0± 78.3pg/ ml) ,TNFα含量 (36 .78± 5 .2 1pm ol/ L ) ,ET1 含量 (75 .2 1± 7.42 ng/ L )显著高于缓解期 (110 .4± 39.2 ,2 8.2 4± 4.16 ,16 4.2 3± 7.2 4,P<0 .0 1)和对照组 (2 6 .70± 16 .2 0 ,14.70± 6 .95 135 .11± 9.81,P<0 .0 1)。缓解期和对照组 IL - 8、TNFα、ET1 之间差异有显著性 ,P<0 .0 1。 IL - 8含量与第一秒钟用力呼气容积占用力肺活量比值 (FEV1 % )、最大呼气中段流速 (MMEF)呈负相关 (r=- 0 .6 2和 r=- 0 .5 6 ,P<0 .0 1)。 TNFα含量和 FEV1 %、MMEF呈负相关 (r=- 0 .71和 r=- 0 .6 3,P<0 .0 1)。 ET1 水平和 FEV- 1% ,MMEF呈负相关 (r=- 0 .81,r=- 0 .82 ,P<0 .0 1)。 结论 IL- 8、TNFα、ET1 和 COPD发病有关 ,并影响肺功能。
Objective To investigate the relationship between interleukin - 8 (IL - 8), tumor necrosis factor α (TNFα) and endothelin - 1 (ET1) and pulmonary function in patients with chronic obstructive pulmonary disease (COPD). Methods Specific radioimmunoassay was used to determine the levels of IL - 8, TNFα and ET1 in 26 cases of acute COPD, 20 cases of remission and 20 cases of healthy controls, and the lung function was measured. Results The levels of IL - 8 and TNFα (36.78 ± 5.21pmol / L) and ET1 (75.21 ± 7.42ng / L) were significantly higher in acute COPD patients In the remission stage (110.4 ± 39.2,2.228.4 ± 4.16, 16.423 ± 7.24, P <0.01) and the control group (26.60 ± 16.20, 14.70 ± 6.95135 .11 ± 9.81, P <0.01). The difference between IL - 8, TNFα and ET1 in remission group and control group was significant, P <0.01. IL - 8 was negatively correlated with FEV1% and MMEF at the first second (r = - 0.62 and r = - 0.56, P <0 .0 1). TNFα content and FEV1%, MMEF was negatively correlated (r = -0.71 and r = - 0.63, P <0.01). The level of ET1 was negatively correlated with FEV - 1% and MMEF (r = - 0.81, r = - 0.82, P <0.01). Conclusion IL-8, TNFα, ET1 and COPD pathogenesis, and affect lung function.
