目的 :分析肝细胞癌在低场 MR的征象。方法 :收集 33例手术病理证实的肝细胞癌的术前低场 MR资料进行分析。结果 :1.肿瘤实质信号均匀区的信号配对主要分为三类 :1T1 WI稍低、T2 WI稍高信号 :18例 ;2 T1 WI等、T2 WI稍高信号 :13例 ;3T1 WI稍高、T2 WI稍高信号 :2例。 2 .肿瘤实质内 MR异常信号区 :1T1 WI低、T2 WI高信号 (斑点状 2 4例 ,片状 9例 ,条状 5例 ) ;2T1 WI稍低、T2 WI高信号 (斑点状 8例 ,片状 2例 ,条状 3例 ) ;3 T1 WI高、T2 WI高信号 (斑点状 4例 ,片状 2例 ) ;4T1 WI低、T2 WI低信号 (斑点状 5例 ,片状 6例 ,条状 2例 )。3.肿瘤边缘在 T2 WI上分三种 :1边缘清楚无分叶 :2 1例 ,其中 11例可以见到“包膜征”;2边缘清楚伴分叶 :5例 ;3边缘不清 :7例。结论 :1低场 MR中 ,肝细胞癌的肿瘤实质在 T1 WI可以是多种信号 ,缺乏特征性 ,但在T2 WI均为稍高信号。 2 MR诊断肝细胞癌一定要有瘤内异常信号。 3肝细胞癌的 MR边缘部的形态应以 T2 WI为准 ,肿瘤边缘可以光滑清楚 ,也可以分叶状或边缘不清 ,“包膜征”有诊断意义
Objective: To analyze the signs of hepatocellular carcinoma at low field MR. Methods: Preoperative low-field MR data of 33 cases of hepatocellular carcinoma confirmed by surgery and pathology were collected for analysis. Signal pairings in the homogeneous region of tumor signal were mainly divided into three groups: 1T1WI slightly lower, T2WI slightly higher signal: 18 cases; 2TWI, T2WI slightly higher signal: 13 cases; 3T1 WI slightly higher , T2 WI slightly higher signal: 2 cases. The signal of abnormal MR in tumor parenchyma: low 1T1WI, high T2WI signal (24 spots, 9 flakes and 5 stripe); 2T1 WI was slightly lower, T2WI high signal (8 spots , 2 cases of lamellar and 3 cases of lamella); 3 high T1 WI and high T2 WI (4 spots and 2 lamella); low 4T1 WI and low T2 WI (5 spots and 6 lamella) Cases, strip 2 cases). 3. Tumor margins in the T2WI divided into three types: a clear edge without lobe: 21 cases, of which 11 cases can see the “capsule sign”; 2 clear edges with lobes: 5 cases; 3 margin unclear: 7 cases. Conclusions: 1 In low-field MR, the tumor mass of hepatocellular carcinoma can be a variety of signals in T1 WI, lacking characteristic, but slightly higher signal in T2 WI. 2 MR diagnosis of hepatocellular carcinoma must have abnormal signal inside the tumor. 3 The morphology of MR margins of hepatocellular carcinoma should be based on T2WI. The tumor margins can be smooth and clear, and the lobes or margins can be unclear. The “capsule signs” are of diagnostic significance
