目的 :评价 MRI对髓外硬膜下肿瘤的诊断价值。方法 :经手术、病理证实的髓外硬膜下肿瘤 6 9例、83个瘤灶 (其中 1例转移癌为随访证实 ) ,回顾性分析其 MRI表现。结果 :( 1)神经源性肿瘤多为 T1 WI低信号、T2 WI高信号 ,T1 / T2 得分比值为 0 .31± 0 .2 ,信号不均匀 ,可见肿瘤血管 ,增强扫描强化明显 ,但不均匀 ;( 2 )脊膜瘤 T1 WI多为等信号或稍低信号 ,T2 WI多为等或稍高信号 ,T1 / T2 得分比值为 0 .85± 0 .19,信号较均匀 ,增强扫描强化均匀 ,增强效果不如神经源性肿瘤高 ,可见脊膜尾征 ;上述肿瘤的 T1 、T2 、T1 / T2 得分比值及强化效果经统计学处理差异有显著性 ;( 3)其它 :脂肪瘤 MRI表现具有特征性 ;转移癌 MRI表现可类似脊膜瘤。结论 :髓外硬膜下肿瘤的 MRI诊断价值极高 ,定位诊断正确率为 98.80 % ,定性诊断正确率为 95 .2 %。
Objective: To evaluate the diagnostic value of MRI for extramedullary subdural tumors. Methods: Sixty-nine patients with extramedullary subdural tumors and 83 tumor foci were confirmed by surgery and pathology (one of them was confirmed by follow-up). The MRI findings were retrospectively analyzed. Results: (1) Neurogenic tumors were mostly low signal on T1 WI and high signal on T2 WI. The ratio of T1 / T2 score was 0.31±0.2, and the signal was not uniform. Tumor vessels could be seen. Uniformly; (2) T1 WI of meningioma is mostly equal signal or slightly low signal, T2 WI is mostly equal or slightly high signal, T1 / T2 score ratio is 0.85 ± 0.19, signal is more even, enhanced scan enhancement Evenly, the enhancement effect is not as high as neurogenic tumors, and the tail tail sign can be seen; the ratios of T1, T2, T1 / T2 scores and enhancement effects of these tumors are statistically significant; (3) Others: MRI findings of lipomas Characteristic; metastatic cancer MRI performance can be similar to meningioma. Conclusion : The MRI diagnosis value of extramedullary subdural tumor is extremely high. The correct rate of localization diagnosis is 98.80 % and the correct diagnostic diagnosis rate is 95.2 %.
