目的:总结合并先天性心脏病的新生儿肺炎的治疗体会。方法:以2012年1月至2014年6月在本院接受治疗的122例肺炎患儿作为研究对象,对照组选择62例新生儿肺炎,不合并先天性心脏病;实验组选择60例合并先天性心脏病的新生儿肺炎,两组患儿都采用常规治疗方法,主要内容包括抗生素抗炎治疗和对症治疗,以及呼吸机治疗。观察并且对比两组新生儿治疗效果及治疗经过。结果:实验组新生儿的并发症情况与对照组新生儿的并发症情况存在差异,P<0.05;实验组新生儿的治疗时间和对照组新生儿的治疗时间存在差异,P<0.05;全部新生儿在治疗后的PH、PaO2、PaCO2、SaO2、心率、呼吸频率要优于治疗前,并且治疗前后存在显著差异(P<0.05)。其中实验组与对照组的患儿在住院天数、呼吸机使用天数、使用呼吸机的人数、并发症发生人数等方面,两组存在明显的差异,P<0.05。讨论:临床诊疗过程中,遇到病情进展快、并发症多、治疗时间长、难度大的新生儿肺炎,首先要考虑该患儿是否合并先天性心脏病,明确诊断,指导治疗。
Objective: To summarize the experience of neonatal pneumonia complicated with congenital heart disease. Methods: From January 2012 to June 2014 in our hospital 122 cases of children with pneumonia were treated as the research object, the control group of 62 cases of neonatal pneumonia were selected, without congenital heart disease; the experimental group selected 60 cases of congenital Neonatal pneumonia with heart disease, both groups of children with conventional treatment methods, the main contents include anti-inflammatory antibiotic treatment and symptomatic treatment, and ventilator therapy. Results: There was difference between the complications of newborns in the experimental group and those in the control group (P <0.05). The neonatal treatment time in the experimental group was different from the neonatal treatment time in the control group (P <0.05) Children after treatment PH, PaO2, PaCO2, SaO2, heart rate, respiratory rate than before treatment, and there was a significant difference before and after treatment (P <0.05). The experimental group and control group had significant differences in the days of hospitalization, the number of ventilator days, the number of ventilators and the number of complications. There was a significant difference between the two groups (P <0.05). Discussion: In the course of clinical diagnosis and treatment, encountered rapid progression of the disease, complications, treatment for a long time, the difficulty of neonatal pneumonia, we must first consider whether the children with congenital heart disease, a clear diagnosis and guide treatment.
