目的 研究抗生素对肺表面活性物质 (PS)功能的影响。方法 2 8只 Wistar大鼠在人工呼吸下行全肺灌洗 (4 0 m l/kg,37℃生理盐水 ) 8~ 10次。当动脉血氧分压 (Pa O2 )降至 13.3k Pa以下时 ,随机分为 4组 (每组 7只 )。 A组 :气管内注入 PS (0 .5 m l,5 0 m g/m l) ;B组 :气管内注入 PS (2 5 mg)与硫酸阿米卡星 (5 m g)混合液 0 .5 ml;C组 :气管内注入 PS(2 5 m g)与硫酸庆大霉素 (2 .5 mg)混合液 0 .5 ml;D组 :气管内注入生理盐水 0 .5 ml。治疗后测定 5~ 15 0 m in的 Pa O2 及潮气量。同时对 A、B、C组经气管所注的液体测定各自的最小表面张力 (γmin)。结果 A、B、C 3组的γmin均在 5 .0 m N/m以下 ,组间无差异 (P>0 .0 5 )。 D组的 Pa O2 在治疗后仍维持在 13.3k Pa以下 ,潮气量也未见增加。 A、B、C组的 Pa O2 在治疗后均迅速上升 ,15 min时达 5 3.3k Pa以上 ,与 D组和治疗前比较呈显著性差异 (P<0 .0 5 ) ,并维持此高值至实验结束。A、B、C组的潮气量在治疗后也显著升高 (与 D组及治疗前比 ,P<0 .0 5 )。结论 阿米卡星或庆大霉素与 PS混合后不影响 PS的活性 ,PS可作为抗生素的载体。
Objective To study the effect of antibiotics on the function of pulmonary surfactant (PS). Methods Twenty-eight Wistar rats underwent total lung lavage (40 ml normal saline at 37 ℃) for 8 to 10 times under artificial respiration. When the arterial partial pressure of oxygen (Pa O2) dropped below 13.3 kPa, it was randomly divided into 4 groups (7 in each group). Group A: Intratracheal injection of PS (0.5 ml, 50 mg / ml); Group B: 0.5 ml of PS (25 mg) and amikacin sulfate Group: Intratracheal injection of PS (25 mg) and gentamycin sulfate (2.5 mg) mixture 0.5 ml; Group D: Intratracheal injection of saline 0.5 ml. PaO2 and tidal volume were measured after 5-1500 mins of treatment. At the same time, the respective minimum surface tension (γmin) was measured for the fluids injected through the trachea in groups A, B and C. Results The γmin values of A, B and C 3 groups were all below 5.0 m N / m, with no difference between the two groups (P> 0.05). In Group D, Pa O2 remained below 13.3 kPa after treatment, with no increase in tidal volume. PaO2 in groups A, B and C increased rapidly after treatment, reaching more than 5 3.3 kPa at 15 min, which was significantly different from that in group D and before treatment (P <0.05), and remained high Value to the end of the experiment. The tidal volume in groups A, B and C also increased significantly after treatment (P <0.05 compared with those in group D and before treatment). Conclusion Amikacin or gentamicin does not affect the activity of PS after mixing with PS, PS can be used as a carrier of antibiotics.
