目的:探讨代谢综合征(MS)患者在不同时期血清肿瘤坏死因了-α(TNF-α)、白介素-6(IL-6)浓度变化的意义以及TNF-α、IL-6与胰岛素抵抗(IR)的相互关系。方法:90例老年肥胖患者根据糖脂代谢紊乱程度分为代谢综合征合并2型糖尿病(T2DM)组、肥胖合并T2DM组、单纯肥胖组各30例。30例健康者为健康对照组。所有受试者均采用酶联免疫吸附法检测血清TNF-α、IL-6水平。结果:①肥胖各组TNF-α、IL-6均较健康对照组明显升高(P<0.01),且TNF-α、IL-6在单纯肥胖组、肥胖合并T2DM组、MS合并T2DM组呈逐渐升高,组间比较差异有统计学意义(P<0.01)。②血清TNF-α、IL-6与稳态模型胰岛素抵抗指数(HOMA-IR)呈正相关关系(P<0.01)。结论:代谢综合征患者血清TNF-α、IL-6浓度与MS的程度相关,可引起胰岛素抵抗的发生。
Objective: To investigate the significance of serum tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in patients with metabolic syndrome and the relationship between TNF-α, IL-6 and insulin resistance IR). Methods: Ninety elderly obese patients were divided into metabolic syndrome with type 2 diabetes mellitus (T2DM) group, obesity combined with T2DM group and simple obesity group with 30 cases in each group. 30 healthy people as healthy control group. All subjects were detected by enzyme-linked immunosorbent assay serum TNF-α, IL-6 levels. Results: ①The levels of TNF-α and IL-6 in obesity group were significantly higher than those in healthy control group (P <0.01), and the levels of TNF-α and IL-6 in simple obesity group, obesity combined with T2DM group, MS combined with T2DM group Gradually increased, the difference between the two groups was statistically significant (P <0.01). ② Serum levels of TNF-α and IL-6 were positively correlated with homeostasis model insulin resistance index (HOMA-IR) (P <0.01). Conclusion: The levels of serum TNF-α and IL-6 in patients with metabolic syndrome are related to the degree of MS, which may lead to the development of insulin resistance.
