目的探讨分析无创正压通气(NPPV)在治疗急性呼吸衰竭(ARF)中的价值。方法将74例各种原因引起的ARF患者随机分为两组,对照组34例,只给予针对病因的标准治疗;NPPV组40例,在标准治疗的同时加用NPPV。通气模式采用压力支持通气(PSV)加呼气末正压(PEEP)。治疗前后测定动脉血气分析及生命体征。结果NIPPV组治疗后2~24 h,动脉氧分压(PaO2)、二氧化碳分压(PaCO2)和pH值较治疗前明显改善(P<0.05),其中PaO2明显升高(P<0.05),PaCO2明显降低(P<0.05),血氧饱和度(SaO2)明显升高(P<0.05)。心率和呼吸频率减慢。结论尽早NPPV能改善急性呼吸衰竭患者的气体交换和生命体征,提高救治成功率。NPPV也可作为首选的通气支持治疗方法。
Objective To investigate the value of noninvasive positive pressure ventilation (NPPV) in the treatment of acute respiratory failure (ARF). Methods A total of 74 patients with ARF caused by various causes were randomly divided into two groups. The control group consisted of 34 patients receiving standard therapy only for the cause of the disease. Forty NPPV patients were treated with NPPV at the same time. Ventilation mode with pressure support ventilation (PSV) plus positive end expiratory pressure (PEEP). Arterial blood gas analysis and vital signs before and after treatment. Results The PaO2, PaCO2 and pH value of NIPPV group were significantly improved (P <0.05), PaO2 was significantly increased (P <0.05), PaCO2 (P <0.05), and the blood oxygen saturation (SaO2) increased significantly (P <0.05). Heart rate and respiratory rate slow down. Conclusion As soon as possible NPPV can improve the gas exchange and vital signs in patients with acute respiratory failure and improve the success rate of treatment. NPPV can also be used as the preferred method of ventilatory support.
