目的探讨社区与农村获得性幽门螺杆菌感染情况,并分析幽门螺杆菌对临床常用药物的耐药性。方法根据患者登记信息,将其分为社区组和农村组,其中社区组206例,农村组263例,分别进行13C-尿素呼气试验检测有无Hp感染;对感染者行胃镜检查,胃黏膜活检培养Hp菌株;用E试验法和琼脂稀释法检测其对甲硝唑、克拉霉素、阿莫西林的耐药性。结果社区组206例患者中有116例感染幽门螺杆菌,感染率为56.31%;农村组263例患者中有211例感染幽门螺杆菌,感染率为80.23%,两组感染率比较,差异具有显著性意义(P<0.01)。社区组Hp菌株对甲硝唑、克拉霉素、阿莫西林的耐药率分别为76.3%、35.9%、11.2%,农村组对甲硝唑、克拉霉素、阿莫西林的耐药率分别为42.5%、22.3%、3.6%,两组对三种常见药物的耐药率比较,差异均具有显著性意义(P<0.01)。结论幽门螺杆菌感染率农村高于城市社区,但是农村患者Hp对甲硝唑、克拉霉素、阿莫西林的耐药率明显低于城市社区患者。
Objective To investigate the prevalence of acquired Helicobacter pylori infection in community and rural areas and to analyze the drug resistance of Helicobacter pylori to commonly used drugs. Methods According to the patient registration information, it was divided into community group and rural group, including 206 community groups and 263 rural groups, and 13C-urea breath tests were performed to detect the presence or absence of Hp infection. Gastroscopy, gastric mucosa Hp strains were cultured by biopsy. The drug resistance of metronidazole, clarithromycin and amoxicillin was tested by E test and agar dilution method. Results Among the 206 patients in the community group, 116 were H. pylori-infected, with an infection rate of 56.31%; 211 of 263 patients in the rural group were infected with H. pylori, the infection rate was 80.23%. The difference in infection rates was significant between the two groups Sexual significance (P <0.01). The resistance rates of Hp strains to metronidazole, clarithromycin and amoxicillin in community group were 76.3%, 35.9% and 11.2%, respectively. The resistance rates of metronidazole, clarithromycin and amoxicillin in rural group were respectively (42.5%, 22.3%, 3.6%). There was significant difference (P <0.01) between the two groups in resistance rates to the three common drugs. Conclusion The infection rate of Helicobacter pylori in rural areas is higher than that in urban areas, but the resistance rate of Hp to metronidazole, clarithromycin and amoxicillin in rural areas is significantly lower than that in urban areas.
