目的调查广州市某钢铁企业高温作业人员代谢综合征(MS)及相关疾病的患病状况,并分析该人群MS的相关危险因素。方法于2008年整群随机抽取广州市某钢铁集团员工进行问卷调查和体检,年龄20~60岁,共4 163人。其中高温作业人员为1 542人,非高温工作人员2 621人。问卷调查包括一般情况、生活行为习惯、疾病史、家族史等。由经过培训的调查员对调查对象进行血压、心率及人体测量,并采集被调查者清晨空腹静脉血,检测血糖、血脂、尿酸等生化指标,对MS的相关危险因素进行Logishic回归分析。结果高温作业人员MS粗患病率为7.52%(男7.49%,女8.33%),经年龄和性别标准化后患病率为8.36%(男8.35%,女4.34%);高脂血症、血压升高、高空腹血糖/糖尿病的标准患病率分别为9.03%、7.64%、5.05%。各患病率均随年龄的增长而升高。年龄、高尿酸血症、白细胞计数、体质指数(BMI)、高血压家族史和缺乏运动是高温作业人员MS的患病危险因素。结论广州某钢铁企业高温作业人员MS患病率低于非高温作业人员,缺乏运动是高温作业人员MS患病的高度危险因素。
Objective To investigate the prevalence of metabolic syndrome (MS) and related diseases among high temperature workers in a steel enterprise in Guangzhou and to analyze the related risk factors of MS in this population. Methods In 2008, a group of Guangzhou Iron and Steel Group employees were randomly selected for questionnaire survey and physical examination, ranging from 20 to 60 years old, a total of 4 163 people. Among them, 1,542 were working at high temperature and 2,621 were non-temperature-related staff. The questionnaire includes the general situation, lifestyle behavior, disease history, family history and so on. Blood pressure, heart rate and anthropometry were measured by trained investigators. Early fasting venous blood samples were collected and blood glucose, blood lipid, uric acid and other biochemical parameters were measured. Logistic regression analysis was performed on the related risk factors of MS. Results The crude prevalence rate of MS was 7.52% (7.49% for males and 8.33% for females), and the prevalence was 8.36% (8.35% for males and 4.34% for females) after age and gender standardization. The prevalence of hyperlipidemia, The prevalence of elevated fasting and high fasting glucose / diabetes were 9.03%, 7.64% and 5.05%, respectively. The prevalence increased with age. Age, hyperuricemia, white blood cell count, body mass index (BMI), family history of hypertension and lack of exercise are risk factors for MS in high-temperature workers. Conclusion The prevalence of MS in high-temperature workers in an iron and steel enterprise in Guangzhou is lower than that of non-high-temperature workers. Lack of exercise is a high risk factor for MS in high temperature workers.
