目的探讨应用地诺前列酮栓治疗足月胎膜早破孕妇的临床疗效。方法选取2015年10月至2016年8月重庆市沙坪坝区妇幼保健计划生育服务中心收治的150例存在引产指征的足月妊娠孕妇作为研究对象,其中胎膜早破100例,未破膜50例(B组)。将100例胎膜早破孕妇根据治疗方式不同分为A组与C组,各50例。A组与B组均使用地诺前列酮栓引产,C组使用缩宫素引产。比较3组孕妇基本资料;记录孕妇宫颈完全成熟、阴道分娩率及剖宫产发生情况;并统计孕妇产褥感染情况及新生儿情况。结果 A组和B组孕妇宫颈完全成熟率均显著高于C组,差异有统计学意义(χ~2=11.11,P<0.01);A、B两组孕妇阴道分娩率差异无统计学意义(P>0.05),而与C组相比则显著升高(χ~2=11.98,P<0.01;χ~2=6.25,P<0.05);A组和B组胎儿窘迫发生率均显著低于C组,差异有统计学意义(χ~2=5.69,P<0.01);A、B两组孕妇产褥感染发生率差异无统计学意义(P>0.05),而与C组相比则显著降低(χ~2=5.316,P<0.01;χ~2=3.843,P<0.05)。结论与缩宫素相比,地诺前列酮栓用于足月胎膜早破孕妇促宫颈成熟效果更佳,引产更加安全有效,剖宫产率也显著降低。
Objective To investigate the clinical efficacy of dinoprostone suppository in the treatment of full-term pregnant women with premature rupture of membranes. Methods A total of 150 full-term pregnant women with induction of labor induction were enrolled in this study from October 2015 to August 2016 in Chongqing Shapingba District Family Planning Service Center. Among them, 100 cases of premature rupture of membranes and 50 cases of unruptured membranes Example (Group B). 100 cases of premature rupture of membranes according to the treatment of pregnant women divided into group A and group C, 50 cases each. Group A and group B were treated with droperidol suppositories, group C using oxytocin. The basic data of 3 groups of pregnant women were compared; the complete maturation of cervix, the rate of vaginal delivery and the incidence of cesarean section were recorded; and the status of puerperal infection and newborns were counted. Results The complete cervical maturation rate of pregnant women in group A and group B was significantly higher than that in group C (χ ~ 2 = 11.11, P <0.01). There was no significant difference in vaginal delivery rate between groups A and B P <0.05), while the incidence of fetal distress in group A and group B was significantly lower than that in group C (χ ~ 2 = 11.98, P <0.01; The difference was statistically significant (χ ~ 2 = 5.69, P <0.01). There was no significant difference in the incidence of puerperal infection between pregnant women in group A and group B (P> 0.05), but in group C (Χ ~ 2 = 5.316, P <0.01; χ ~ 2 = 3.843, P <0.05). Conclusions Compared with oxytocin, droperidol suppository is more effective for cervical maturation in term preterm premature rupture of membranes, induction of labor is more safe and effective, and cesarean section rate is significantly lower.
