目的观察新型鼻塞持续正压通气(NCPAP)与常规机械通气(CMV)对新生儿呼吸窘迫综合征(NRDS)的治疗效果,NCPAP是否降低呼吸机相关性肺炎(VAP)、呼吸机所致的肺损伤(VILI)的发生。方法将2006年4月—2008年2月34例确诊为NRDS的患儿随机分为两组,一组为CMV组,另一组为NCPAP组。CMV组应用美国产VIP-BIRD呼吸机(IPPV+PEEP),NCPAP应用德国STEPHAN呼吸机,观察两种方法治疗NRDS的疗效情况。结果两组患儿在上机时间、存活率方面差异无显著性,但在呼吸机并发症方面差异有显著性(P<0.05)。结论正压通气;机械通气;呼吸窘迫综合征;新生儿
Objective To observe the effect of NCPAP and conventional mechanical ventilation (CMV) on neonatal respiratory distress syndrome (NRDS), whether NCPAP reduces ventilator-associated pneumonia (VAP), ventilator-induced lung Injury (VILI) occurred. Methods From April 2006 to February 2008, 34 children diagnosed with NRDS were randomly divided into two groups: one was CMV group and the other was NCPAP group. In the CMV group, the American-made VIP-BIRD ventilator (IPPV + PEEP) was used. NCPAP was applied to the German STEPHAN ventilator to observe the curative effect of the two methods on NRDS. Results There was no significant difference in the time and survival rate between two groups, but there was significant difference in the complication of ventilator (P <0.05).
