目的探讨电视胸腔镜(VATS)下食管癌手术的方法。方法对12例食管癌经上腹切口游离胃,制成管状胃,并经胸骨后上提至左颈完成食管胃吻合,改侧卧位胸腔镜下经右胸游离食管、肿瘤,清扫淋巴结。结果全组无手术死亡,手术时间180~240 min,胸腔镜游离食管时间60~110 min,胸腔引流量120~350 ml,术后住院13~17 d,术后吻合口瘘1例。结论 VATS下经胸骨后途径食管癌根治术,安全性、可行。
Objective To discuss the method of esophagectomy under VATS. Methods 12 cases of esophageal cancer by the upper abdominal incision free stomach, made of tubular stomach, and after the suprasternal bone raised to the left neck to complete esophageal anastomosis, lateral recumbent thoracoscopic free right esophagus, tumor, lymph node dissection. Results There was no operative death in all patients. The operative time was 180-240 min. The time of esophageal esophageal esophagectomy was 60-110 min. Thoracic drainage was 120-350 ml. The postoperative hospital stay was 13-17 days. One case was anastomotic fistula. Conclusions Radical resection of esophageal carcinoma by trans-sternal approach under VATS is safe and feasible.
