目的了解厦门市预防接种门诊免疫规划服务能力,对存在的问题提出解决对策。方法采用问卷调查方式,从基本情况、人员配备、工资待遇等情况,对2015年全市39家预防接种门诊进行调查研究。结果厦门市共有39家预防接种门诊,覆盖每个街道/乡镇。接种门诊服务面积以<30平方公里为主,占61.54%,服务人口以>10万为主,占53.85%,平均服务人口为11.14万人。接种门诊共有预防接种工作人员325人,以35岁以下(69.85%)、本科及以上学历(38.46%)、护理专业(84.31%)为主。厦门市预防接种工作人员配比为0.75人/万人,最高的为同安区汀溪卫生院(2.43名/万人),最低的为同安区大同街道社区卫生服务中心(0.23名/万人),仅有6个接种门诊达到1人/万人。全市接种门诊每年人均接种针次最多的为同安区大同街道社区卫生服务中心(14 858针次),最少的为思明区鼓浪屿街道社区卫生服务中心(1 285针次),共有14家接种门诊每年人均接种针次数超过6 000针次,占35.90%。全市接种门诊在实施绩效考核后,工资较之前增加的有27家(69.23%);与同单位医疗部门相比,预防接种人员工资较之低的有11家(28.20%);预防接种人员工资收入与创收有关的有14家(35.90%),结核病管理有奖金的有14家(35.90%)。结论厦门市预防接种门诊存在数量无法满足群众需求、工作人员严重不足、人员工资待遇与工作量不对等的问题,应通过增设接种门诊、科学设置接种门诊工作人员岗位数量、扩充工作人员以减轻工作量、保障预防接种工作人员的工资待遇、提高工作积极性等措施提高预防接种服务质量与能力。
Objective To understand the immunization planning service capacity of outpatient vaccination in Xiamen and propose solutions to the existing problems. Methods A questionnaire survey was carried out to investigate and survey 39 vaccination clinics in 2015 from the basic conditions, staffing and wages. Results There were 39 vaccination clinics in Xiamen City, covering every street / town. Inpatient outpatient service area of <30 square kilometers, accounting for 61.54%, service population> 100,000, accounting for 53.85%, the average service population of 111400 people. The vaccination clinics in the city received the highest number of inoculation per person per year for Tongchuan Datong Street Community Health Service Center (14,858 stitches), at least the Gulangyu Street Community Health Service Center (1,285 st passes) in Siming District and a total of 14 vaccination clinics The number of inoculation per person per year surpassed 6,000 needles, accounting for 35.90%. The city’s vaccination clinics in the implementation of performance appraisal, the wage increase over the previous 27 (69.23%); with the unit with the medical sector, vaccination staff salaries lower than the 11 (28.20%); vaccination staff salaries There were 14 (35.90%) income-generating and 14 (35.90%) tuberculosis-related bonuses. Conclusions The number of inpatient vaccination clinics in Xiamen City can not meet the needs of the masses, the staffs are seriously deficient, the staff salary and the workload are not equal. The number of outpatient clinic staff should be set up scientifically through additional vaccination clinics to expand the staff to reduce the workload Quantity, protection of vaccination staff wages, improve work enthusiasm and other measures to improve the quality and ability of vaccination services.
