Insulin-dependent diabetes mellitus (IDDA) is caused by a lack of endogenous insulin due to islet beta-cell dysfunction. The reason for the obstacles of autoimmune mechanism and in a specific genetic background, based on environmental factors acting on the individual and disease. This article provides an overview of the characteristics of Japanese IDDM, human leukocyte antigen (HLA) genes, pancreatic autoantibodies and their associated antigenic and autoimmune disorders and the authors’ experience. First, IDDM and HLA gene IDDM consistent in a twins accounted for about 30 to 50%, higher than about 10% of the twins twins, suggesting the existence of the role of genes. Specifically, it is associated with HLA genes that are the backbone of human adaptive host (MHC) complexes. The gene is located in the human sixth gene
