目的探讨不同剂量卡铂动脉灌注化疗子宫颈癌组织内铂浓度的分布规律。方法选择经病理学证实的中晚期宫颈癌患者10例,分成A、B两组,分别按300mg/m2、200mg/m2的剂量经双侧子宫动脉灌注卡铂。于灌注卡铂后0、10、20和30min,取新鲜活体癌组织0.3~0.5g,以原子吸收光谱法测量铂离子浓度。结果①A、B两组动脉灌注后宫颈癌组织内铂离子浓度的高峰值均出现在化疗结束后即刻,且随时间的延长而快速下降,呈下降曲线;②动脉灌注卡铂后0、10、20、30min,A组宫颈癌组织内铂离子浓度分别是B组的2.93倍、1.87倍、2.25倍、1.96倍,差异均有统计学意义(P<0.05);③两组从动脉灌注开始至30min铂离子浓度的给药时间曲线下面积,A组是B组的2.27倍,差异有统计学意义(P<0.05)。结论子宫颈癌行动脉灌注化疗时,在卡铂≤300mg/m2的剂量范围之内,癌组织内的铂离子浓度随着卡铂剂量的增加而成倍数地增加,宫颈癌组织对卡铂无吸收饱和度。
Objective To investigate the distribution of platinum concentration in cervical cancer with different doses of carboplatin arterial infusion chemotherapy. Methods Ten patients with advanced cervical cancer confirmed by pathology were divided into A and B groups and received carboplatin via bilateral uterine arterial infusion at 300mg / m2 and 200mg / m2, respectively. At 0, 10, 20, and 30 min after the instillation of carboplatin, 0.3-0.5 g of fresh live cancer tissue was taken and the platinum ion concentration was measured by atomic absorption spectrometry. Results ① The peak values of platinum ion concentration in cervical cancer tissues after A and B arterial instillation both appeared immediately after chemotherapy, and decreased rapidly with the prolongation of time. (2) After arterial infusion of carboplatin at 0, 10, The concentration of platinum ion in group A was 2.93 times, 1.87 times, 2.25 times and 1.96 times higher than that in group B at 20 and 30 minutes, respectively (P <0.05) The area under the curve of administration time of 30min platinum ion concentration was 2.27 times of that of group B, the difference was statistically significant (P <0.05). Conclusions When intra-arterial infusion chemotherapy of cervical cancer is performed, the concentration of platinum ion in cancer tissue increases with the increase of carboplatin dose within the dose range of carboplatin ≤300mg / m2, while the rate of carboplatin in cervical cancer tissue Absorb the saturation.
