Morbid obesity has reached a dramatic dimension in the western world.Obesity is associated with many co-morbidities that require surgery.Consequently, many of the obese patients have to undergo general anesthesia.Due to physiologic changes of the obese patient, the anesthesiologist faces specific challenges.Specifically, the cardio-vascular and the respiratory systems are compromised and require particular anesthesia care to carry the obese patient through surgery safely.Another challenge is the potentially difficult airway.A secure airway is of utmost importance during general anesthesia.Thorough assessment of the airway and meticulous planning are mandatory to minimize the risk of airway disaster in this group of patients.Obstructive sleep apnea (OSA) is associated with morbid obesity.OSA poses an additional risk in the postoperative phase.OSA patients should be treated with low doses of narcotics and should receive a continuous positive airway pressure mask (CPAP) to avoid the risk of apnea and hypoxia.
