Inflammatory myofibroblastic tumor (IMT) was first described in 1937 as a primary lung tumor.In the gastrointestinal tract, it occurs very rare and the diagnosis is especially based on immunohistochemistry (IHC).In the colon, we have been reported the 25th case of IMT.In the IMT of the colon, clinical endoscopic and gross feature can imitate a carcinoma or a GIST.It is usually a protruded mass, with or without ulceration, sometimes covered by normal mucosa,with a shallow depression.Microscopically, it consists in proliferation of spindle cells with fascicular arrangement admixed with inflammatory cells, predominant lymphocytes and plasma cells.The IHC pattern of IMT is quite specifically and can offer informations about its histogenesis.The spindle cells, without atypia, express Vimentin,SMA, Desmin and ALK and are CD34 and CD117 negative.On the other hand, CD34 stain reveals an intense angiogenesis, with predominance of immature vessels.CD 117 expression can be observed in the endothelial cells of the neoformed vessels.Based on this immunophenotype, we hypothesize that this tumor seems to occur from the the pluripotent stromal cells, CD34 positive.These cells can be either differentiated in the interstitial cells of Cajal (telocytes), which are also CD34 positive or, during differentiation, can loss CD34 positivity.Depend on the type of differentiation, GIST or IMT can result.Despite of losing CD34 and CD117 expression in the tumor cells, these two markers seems to be present in the endothelial cells of the neoformed vessels.Because in both stomach and colon the interstitial cells of Cajal significantly decrease with age, our hypothesis could explain the predominance of IMT in children and young adults.Acknowledgement: This work was partially supported by the Romanian National University Research Council (CNCSIS), Ministry of Education and Research, projects frame: PN Ⅱ-PD, no.504/2010 and by the Sectoral Operational Programme Human Resources Development, financed from the European Social Fund and by the Romanian Government under the contract number POSDRU/89/1.5/S/60782.
