  Aim: To observe urine metabolic changes after Xinfeng capsule treatment in knee osteoarthritis (KOA) rats using gas chromatography time-of-flight mass spectrometry (GC-TOF/MS) and explore its possible mechanism.Methods: Rats were randomly divided into normal (n =8), model (n =8), and Xinfeng capsule groups (n =8).Establishment of the KOAmodel was accomplished with a 20μLinjection of 4% papain and 0.03 mol/L L-cysteine solution into the right knee of rats on days 1, 3, and 7.After model establishment for 14 days, rats in the Xinfeng group were given a gastric gavage of Xinfeng capsule (0.375g/kg) daily for 30 days.The normal and model groups were given the same normal saline (NS) as that of the Xinfeng capsule group.After the final dose, 12 h urine from all rats was collected.Cartilage damage and ultrastructural changes were observed by HE staining.Using GC TOF/MS combined with principal component analysis, partial least squares discriminant analysis, orthogonal partial least squares discriminant analysis,and heat maps, changes in metabolic spectrum were analyzed to identify potential biomarkers and analyze related metabolic pathways.Results: Compared with the model group,the Xinfeng capsule group had less cartilage damage.Twenty-three potential biomarkers were found in the model group, including alanine, 5-amino pentanoic acid, asparagine,urine acid, N-carbamylglutamate, and glutamine.The Xinfeng capsule group had more normal levels of these biomarkers compared with the model group.Conclusion: Xinfeng capsule has a therapeutic effect on KOArats and its mechanism may be related to the regulation the metabolism of amino acids, nucleic acids, fatty acids, and vitamin B6.
