Estrogen synthesis in both benign and malignant human breast tissue is complex, involving several enzyme systems with considerable inter-patient variability.The most important enzymes are aromatase (CYP 19), 17-hydroxysteroiddehydrogenase (with several isoforms) and the sulfatase/sulfotransferase-system.We and others have recently published novel findings underlining the impact of CYP19 as well as 17-HSD isoforms on intra-tumor endocrinology.These findings will be presented.In addition, recent publications suggest the ERα to be a major player in human breast cancer intracrinology, although the precise mechanisms are unknown at this stage.Thus, different models explaining the impact of the ER on tumor estrogen levels will be discussed.Manipulation of intra-tumor estrogen levels has been used for several decades for the treatment of both early and advanced breast cancer.
