高血压患者新发糖尿病的降糖药物治疗分析1例

一项横断面研究表明，在我国原发性高血压门诊患者中，已确诊和新检出的糖尿病总患病率高达24.3％，其中新检出的糖尿病占34.7％.与单纯高血压患者相比，内皮细胞和血管功能受损更加严重，动脉硬化和动脉粥样硬化常见，伴糖尿病的高血压患者发生心血管疾病的风险至少增加1 倍.因此，早期筛查和治疗高血压患者合并的糖尿病，有助于降低心血管疾病风险[1].而T2DM 患者常合并CKD.糖尿病可使CKD 的发生风险

目的：分析患者抗癫痫药物血药浓度过低的影响因素，临床药师为医师提出更换药物建议，在临床治疗中发挥了作用。方法：临床药师在临床跟踪患者，关注患者用药期间的疗效和不良反应，协助医师分析患者血药浓度过低的原因，为医生提出建议，为患者提供合理的药学服务。结果：临床药师参与的药物治疗过程，及时发现潜在的治疗风险，提高了患者用药的疗效。结论：药师参与临床药物治疗实践，有利于提高临床药物治疗水平。

1例59 岁男性癫痫者，给予抗癫痫药物卡马西平，出现白细胞、中性粒细胞较用药前明显降低，根据时间相关性并排除其他药物，考虑为卡马西平不良反应。临床药师建议立即停药，改用丙戊酸钠治疗，医生予以采纳，更换抗癫痫药物，患者出院后随访，复查血常规回复正常。
