Transcatheter Aortic Valve Implantation: Percutaneous and Subclavian Access Techniques

　　Transcather Aortic Valve Implantation (TAVI) is an alternative for surgical aortic valve replacement in selected patients with severe co-morbidity or is inoperable.We use the self expandable 18 french stented valve (CoreValve) in 60 to 80 cases a year.Patient selection is done with strict critera: 1 symptomatic severe aortic stenosis, 2 inoperable or high risk patients, 3 evaluation of the coronary arteries, aortic arch and sinus, subclavian artery and iliac and femoral arteries.The interventional cardiologist and vascular surgeon decide together which strategy is the best for the individual patient, percutaneous transfemoral, or surgical through the subclavian artery depending on the diameter, angulation and calcification of the iliac and femoral arteries.
