Objective:To investigate the diagnosis and surgical treatment of traumatic upper cervical instability.Methods:Forty three cases of upper cervical instability were reviewed.According to imaging classification,there were 13 cases of fresh dentoid fractures,22 cases of old dentoid fractures,1 case of Jeffersons fracture,6 cases of rupture of transverse ligament.Forty one cases were accepted surgical treatment.The operative modes included anterior AO midheaven cancellous screw fixation (12 cases),posterior Gallie or Brooks fixation (11 cases),Apofix fixation (5 cases),occipital cervical fusion and fixation (9 cases),C1~2 pedicle screw-rod fixation(2 case),C1~2 lateral mass screw fixation (1 case),Halo external fixation (1 cases).
