Intraoperative Transesophageal Echocardiography (TEE) is an important monitor in cardiovascular surgery, mainly in congenital heart procedure (Class 1) as suggested by the American Society of Anesthesiologists and the Society of Cardiovascular Anesthesiologists (SCA).However, a complete diagnosis of the congenital heart lesion (cardiac catheterization, transthoracic echocardiography, cardiac magnetic resonance imaging or multislice computer tomography) should be defined prior to performance of intraoperative TEE.The congenital heart procedure is associated with potential residual shunts, valvular regurgitation, obstruction or myocardial dysfunction. The intraoperative TEE can monitor the ischemia-reperfusion injury associated with down-regulation of beta receptors in the pos by-pass period and the cardiac performance during the weaning of cardiopulmonary bypass. Moreover, guide the pharmacological therapy to improve heart function.Using the TEE doppler and the color doppler, is possible to measure the pulmonaryto-sistemic flow ratio and the residual valvular regurgitation or obstruction. The intraoperative TEE can provide a better outcome in congenital heart repair
