扩张后胸三角皮瓣修复面颈部瘢痕

  目的:探讨面颈部烧、烫伤后瘢痕的修复方法。方法:本组102例患者均采用胸三角皮瓣预扩张后,带蒂转移修复面颈部瘢痕。据修复缺损面积的不同采用单侧或双侧胸三角皮瓣预扩张,在胸三角皮瓣下靠近肩部最好植入一个扩张器,切口选择在锁骨下;注水不宜过急,以免妊娠纹产生。术中先将面部瘢痕切除,松解后,使眼、口、鼻恢复到正常解剖位置后,据缺损大小设计皮瓣。可将面部瘢痕皮瓣翻转与胸三角皮瓣蒂部形成铰链而封闭蒂部创面,不必外用石膏固定。三周后将皮瓣延迟后断蒂修复剩余的面颈瘢痕。结果:本组102例,应用扩张后胸三角皮瓣转移修复面颈部瘢痕,效果满意。结论:扩张后胸三角皮瓣是修复较大面积面颈部瘢痕的有效方法。
