Worldwide, the prevalence of cardiovascular disease continues to grow.In the western world, given all the advances in medical therapy, coronary stenting and the expanding indications for surgical heart failure therapy (right and left ventricular assist devices and the total artificial heart), it is growing increasingly common for the anesthesiologist to manage very old (age 85 and above) and fragile patients with significant, medically treated disease for non cardiac surgery, both emergency and elective.Guidelines for the perioperative management of these complex patients have been recently modified in the United States by the American College of Cardiology and American Heart Association.This (45 minute) lecture aims to address these issues and provide the anesthesiologist and perioperative physician basic guidelines for the management of patients with cardiac disease in the perioperative period, with the ultimate goal being the avoidance of cardiovascular complications.
