Acute Effect of Whole-body Periodic Acceleration on Coronary Microcirculation and Insulin Resistance

  Background.Exercise training is beneficial in the primary and secondary prevention and treatment of coronary artery disease and a widening variety of other chronic diseases.However, moderate aerobic exercise for at least 30 min a day for at least five days is not attainable in patients who are incapable or unwilling to meet these requirements such as those with neurological and rheumatological diseases, mobility problems, and frailty among others.Whole-body periodic acceleration (WBPA) can serve as an alternative or complement to certain therapeutic benefits of exercise.The increases of pulsatile shear stress to the endothelium lead to the release of nitric oxide from stimulation of endothelial nitric oxide synthase (eNOS) and improvement of the endothelial function.The purpose of this study was to investigate the acute effect of WBPA on coronary flow reserve (CFR) through transthoracic Doppler echocardiography (TTDE) and insulin resistance in type 2 diabetic patients.Methods.This study consisted of 20 patients with type 2 diabetic patients who underwent CFR examination before and immediately after WBPA.The flow velocity in the distal portion of the left anterior descending coronary artery (LAD) was measured with TTDE at baseline and during adenosine infusion.CFR was calculated as the ratio of hyperemic to basal mean diastolic flow velocity.Results.WBPA increased CFR from 2.50.6 to 2.90.7, and decreased serum insulin from 27.415.5 to 21.716.3 and HOMA-IR from 12.37.8 to 10.49.2, respectively (all p<0.05).Coronary angiography showed significant LAD narrowing in 11 of the 20 patients, but WBPA increased CFR from 2.30.4 to 2.50.4 in them as well (p<0.05).Conclusions.This study demonstrates that WBPA improves coronary microcirculation and insulin resistance in type 2 diabetic patients.
