Background Stage Ⅳ or M1 gastric cancer is generally considered ineligible for surgery with curative intent.The Japanese committee for revising the national treatment guidelines has been seeking for exceptions to the rule, and patients with technically resectable metastasis to the para-aortic region has been considered as one of candidates for multimodality treatment that includes radical surgery through trials conducted by Japan Clinical Oncology Group (JCOG).
