More than 90% of pregnant women in the tropical northern areas of Australia have optimal levels of vitamin D, as demonstrated by our initial study.This is in contrast to similar studies conducted in the southern regions of Australia where the climate is cooler.These showed up to 50% of their local antenatal population to be vitamin D insufficient or deficient.The substantial differences in the measured vitamin D level between pregnant women living in the southern and northern parts of the country polarizes opinion as to whether a national vitamin D screen should be introduced for all pregnant women in Australia.Evidence of association between suboptimal vitamin D levels and adverse pregnancy outcomes is accumulating.The risks ofpre-eclarnpsia, emergency caesarean section and low infant birth weight are increased in pregnancies with inadequate vitamin D levels.Also, suboptimal vitamin D states are common in women with gestational diabetes, and are correlated with poorer glycaemic control.One study in Sydney reported 40% of pregnant women with gestational diabetes were low in vitamin D.Further investigation into vitamin D levels of women living in northern Australia, in particular in those with pre-existing diabetes or gestational diabetes, is warranted to decide whether a universal antenatal vitamin D screen should be imposed in the country.
