目的 分析常州市小儿热性惊厥流行病学特点及转为癫痫的危险因素.方法 回顾性分析2013年1月~2015年12月我院收治的100例小儿热性惊厥患儿的临床资料,按照是否转为癫痫分为癫痫组与非癫痫组,总结小儿热性惊厥流行病学特点,分析小儿热性惊厥转为癫痫的危险因素.结果 2013年~2015年,常州市小儿FC 发病率呈逐渐上升趋势,以2013年发病率(22.00%)最低,2015年发病率(43.00%)

目的 分析常州市小儿热性惊厥流行病学特点及转为癫痫的危险因素.方法 回顾性分析2013年1月~2015年12月我院收治的100例小儿热性惊厥患儿的临床资料,按照是否转为癫痫分为癫痫组与非癫痫组,总结小儿热性惊厥流行病学特点,分析小儿热性惊厥转为癫痫的危险因素.结果 2013年~2015年,常州市小儿FC 发病率呈逐渐上升趋势,以2013年发病率(22.00%)最低,2015年发病率(43.00%)最高;本组小儿FC 以男性多见(68.00%),发病年龄以6个月~3岁居多(71.00%),发病季节以冬(48.00%)、秋(29.00%)为主,原发病多为上呼吸道感染(57.00%),脑电图异常所占比例为39.00%,首次惊厥发作类型以全身性多见(83.00%),体温多在38.9℃以内(88.00%),首次惊厥前发热时间大部分均<24h(87.00%),首次惊厥次数为1~3次所占比例居多(77.00%),惊厥持续时间<20min 所占比例较高(78.00%),分型以CFC 为主(71.00%),围生期异常所占比例为13.00%;本组100例小儿FC,随访继发癫痫8例,发生率为8.00%;癫痫组年龄小于非癫痫组(P<0.05),其有FC 家族史、脑电图异常、CFC 所占比例均高于非癫痫组(P<0.05),同时癫痫组惊厥次数、惊厥持续时间均高于非癫痫组,对比差异有统计学意义(P<0.05).结论 惊厥次数>3次、脑电图异常、CFC 型惊厥均为小儿FC 转为癫痫的独立危险因素,积极控制上述危险因素,并予以预防性治疗,可降低癫痫发生风险.
