Introduction: Basaloid squamous cell carcinoma of the esophagus (BSCCE) is a rare disease with no standard treatment.The aim of this study is to explore the treatment modality and prognosis of BSCCE.Methods: A total of 142 patients with BSCCE that received treatment in our hospital from March 1999 to July 2010 were retrospectively analyzed.All patients received surgery and survival analysis was completed by the Kaplan-Meier method and Log-rank test.Prognostic factors were identified by Coxs hazard regression model.Results: There were 26 patients included in stage,60 in stage, 53 in stage and 3 in stage.The median survival time (MST) was 32 months, 1-,3-and 5-year OS were 81.4%,46.8% and 31.0%; respectively.The survival of stage were significantly longer than that of the stages (p<0.001).The MST and 5-year OS of patients with radical resection were significantly better than that of the palliative resection (p<0.001).The MST and 5-year OS of the patients with tumors located in the lower thoracic and upper/middle esophagus were 59.0 months and 47.4 % vs 27.0 months and 18.1%; respectively (p-0.002).However, the survival was not significantly improved by radiotherapy or chemotherapy.TNM stage, operation type and tumor location were found to be independent prognostic factors.Distant metastasis was the main failure pattern in 71 cases(63/71) and radiotherapy reduced the locoregional recurrence rate (p=0.024).Conclusions: BSCCE is a distinctive disease with the propensity of distant metastasis.Multimodality therapy including surgery and radiotherapy should be recommended.Radical resection in earlystage patients might achieve good results.However, chemotherapy requires further exploration.
