  The distinction of atypical ductal hyperplasia (ADH) from low grade ductal carcinoma in-situ (DCIS) can be sometimes difficult and problematic.Both qualitative and quantitative criteria are needed before a diagnosis can be made.Low grade DCIS (WHO, 2003) architecturally shows sharply punched out cribriform spaces, polarizationin in the lumen and club shaped micropapillae; cytologically, it is composed of a monotonous, uniform rounded cell population with subtle increase in nuclear to cytoplasmic ratio, round nuclei with or without hyperchromasia, which involve at least two duct spaces or more than 2-3 mm in aggregate dimension.
