Neurosurgical diseases like brain tumors and vascular malformations as well as spinal tumors usually become symptomatic and require microsurgical removal.However in some cases these pathologies are found incidentally or surgical removal carries a high risk of neurological deterioration after the surgical procedure.There always has to be an individual decision together with the patient and relatives which is the best way to proceed either with operation or observation.Examples of benign intracranial tumors and cavernous malformations as well as spinal intradural tumors will be presented which were primarily treated in a conservative fashion.
