  Purpose: Our previous study indicated that percutaneous stereotactic thermocoagulation of the second thoracic ganglionectomy and sympathectomy (T2) were effective treatment for palmar hyperhidrosis (PH) that provided excellent immediate and long-term results, as well as a low complication rates.However, this treatment can cause severe compensatory hyperhidrosis (CH).Some recent articles advocated T4 orT3 sympathectomy for PH to minimized CH.In this study, percutaneous stereotactic thermocoagulation of the fourth or third thoracic ganglionectomy and sympathectomy (T4 or T3) was performed to treat PH.We investigated the occurrence and severity of CH.Methods and results: Between June 2011 and June 2013, percutaneous stereotactic thermocoagulation of T4 orT3was performed in 81 Chinese patients with palmar hyperhidrosis under local anesthesia.Seventy patients were followed between 1 to 17 months after the procedure.Sixty-two patients had dry or nearly dry hands, 7 patients had wet hands, and 1 hand was false positive in T4 sympathectomy.Sixty-one patients had no or mild CH, 9 patients had moderate CH, and no patient had severe CH.Three hands (2.1%) experienced recurrent PH, and one side (1.5%) developed pneumothorax.Sixty-five patients were satisfied with the outcome, three patients were partially satisfied and 3 patients were dissatisfied in T4 sympathectomy, and no patient expressed regret.Conclusions: Percutaneous stereotactic thermocoagulation ofT4 or T3 is a safe and effective treatment for palmar hyperhidrosis.This method also minimizes compensatory hyperhidrosis.
