目的:研究白细胞VCS参数(Volume,体积;Conductivity,电导;Scatter,散射)在慢性乙型肝炎病毒感染人体后不同发病期的变化及临床意义。方法:182例分成四组,其中正常对照组48例、乙肝携带组46例、乙肝后肝硬化组49例、乙肝后肝癌组39例,运用生化分析仪Olympus AU2700检测丙氨酸氨基转移酶(ALT),运用血球分析仪BackmanCoulter LH750检测白细胞VCS参数,比较四组间白细胞VCS各参数的结果,并分析VCS各参数与ALT相关性。结果:与对照组比较,乙肝携带组MNV、MLS、MMS的变化存在显著性差异(P<0.05~0.01),乙肝后肝硬化组、乙肝后肝癌组白细胞VCS参数逐渐变大有MNV、MNV-SD、MLV、MLV-SD、MNC-SD、MLC-SD、MLS-SD、MMS-SD(P<0.05~0.01),逐渐变小有MMV、MNC、MLC、MNS、MLS、MMS(P<0.05~0.01);与乙肝后肝硬化组比较,乙肝后肝癌组参数MLV-SD、MLC、MLS、MMS、MMS-SD变化存在显著性差异(P<0.05~0.01);与ALT存在正相关(P<0.05)参数有MNV、MNV-SD、MNC-SD、MLV-SD、MLS-SD。结论:白细胞VCS参数在乙型肝炎病毒感染人体后不同发病期的变化可以为乙肝疾病进展提供新的实验室指标。
OBJECTIVE: To study the changes and clinical significance of VCS parameters (Volume, volume, Conductivity, conductance, Scatter, scattering) of leukocytes during different stages of the disease after chronic hepatitis B virus infection. Methods: One hundred and eighty-two patients were divided into four groups, including 48 normal control subjects, 46 hepatitis B carriers, 49 hepatitis B liver cirrhosis patients and 39 hepatitis B hepatocellular carcinoma patients. The results of the parameters of leukocyte in four groups were compared. The correlation between VCS parameters and ALT was analyzed. Results: Compared with the control group, the changes of MNV, MLS and MMS in HBV-carrying group were significantly different (P <0.05-0.01). The VCS parameters of white blood cells in patients with post-hepatitis B cirrhosis and HBV- (P <0.05), and gradually decreased with MMV, MNC, MLC, MNS, MMS and MMS (P <0.05) ~ 0.01). There were significant differences in MLV-SD, MLC, MLS, MMS and MMS-SD parameters between patients with posthepatitic cirrhosis and those with hepatitis B and cirrhosis (P <0.05-0.01) <0.05) Parameters are MNV, MNV-SD, MNC-SD, MLV-SD, MLS-SD. CONCLUSION: The changes of VCS parameters of leukocytes during different stages of infection after hepatitis B virus infection can provide a new laboratory index for the progress of hepatitis B disease.
