Rurpose To demonstrate a successful delivery after the transfer of re-cryopreserved blastocysts when vitrified-thawed cleavage embryos was cultured to blastocysts in the event of an unsuccessful pregnancy attempt of frozen embryo transfer.Methods And Results A 33-year-old woman underwent a long-treatment protocol for ovarian stimulation.Fifteen mature oocytes were obtained, and ten were fertilized.On day 3, two cleaved embryos were transferred, but no implantation occurred.The remaining embryos were vitrified.Subsequently, five vitrified day-3 embryos were thawed and cultured to blastocyst.Four of five embryos developed to the blastocyst stage.Two of the four blastocysts (3BA, 3AA) were transferred at day-5.The remaining embryos were continued to be cultured and two supernumerary good blastocysts (4BB, 4BC) remained unexpectedly, and they were refrozen by closed-system vitrification (J.Y.Laboratory Inc., medium and leaf) under consent on day 6.Subsequently, the re-vitrified day-6 blastocyst was transferred.The woman became pregnant and delivered a healthy male.Conclusions The transfer of refrozen supernumerary embryos resulted in successful pregnancy and delivery.We will continue to investigate the safety and efficacy of the elective freezing of all embryos as well as their refreezing so that supernumerary embryos can be reused to avoid ovarian hyperstimulation syndrome or in cases of repeated failed pregnancies.
