PURPOSE: To report a patient presenting with necrotizing scleritis, peripheral ulcerative keratitis and optic neuritis ,in the meanwhile, the patient is associated with both rheumatoid arthritis and syphilis.METHODS: A seventy-year-old man with rheumatoid arthritis and syphilis who developed necrotizing scleritis, peripheral ulcerative keratitis and optic neuritis of the right eye was referred to our hospital with a 2-week history of red, painful eye and blurred vision on 2013 September.He had a 10-year history of rheumatoid arthritis, treated with oral prednisone 10mg/d and leflunomide 10mg twice a day.He complained constant joint stiffness, joint swelling, particularly over his fingers and knees.
