A 42-year-old man was referred to the hospital because of persistent atypical chest pain.He gave a history of thoracic trauma in a motocross accident six months previously.Transthoracic echocardiography (TTE) at the time of his presentation showed dilatation of the origin of the right coronary artery (RCA) consistent with a pseudoaneurysm, with areas of increased echodensity consistent with thrombus.Coronary artery computed tomography (CTA) showed a partially thrombosed pseudoaneurysm adjacent to the right atrioventricular groove.Cardiothoracic surgery showed a giant RCA pseudoaneurysm containing thrombus.The pseudoaneurysm was excised and the RCA underwent coronary artery bypass grafting.Transthoracic echocardiography is capable of locating acoronary pseudoaneurysm of the RCA, in addition to estimating its size and detecting the presence of thrombus within the pseudoaneurysm.
