三维可视化体外无创定向血管消融栓塞的实验研究

  目的 初步探索通过HIFU实现体外三维可视化定向血管消融栓塞的可行性.方法 选取小型猪肝脏的为研究对象,采用前期完成的MRI三维重建与实时B超图像融合及配准方法,选取融合图像上可见而实时超声图像上不能识别的小动脉进行HIFU定向消融;通过声诺维明确靶区血供缺失后停止治疗.术后手术切取靶区组织,肉眼及显微镜下观察靶区.结果 在三维可视化系统帮助下,通过HIFU成功消融小型猪肝脏三支直径约2mm的小动脉;肉眼见靶灰白色凝固性坏死,其周围见暗红色充血区,再外围为苍白色反应带,最大截面分别为22.6mm2; 26.4mm2;19.8 mm2;显微镜下见靶区肝内微血管内皮严重破坏,血管壁塌陷,肝板破坏,肝细胞核固缩.结论 精确图像融合的三维可视化与HIFU技术结合,可实现体外无创的定向血管消融,达到栓塞目标血管的作用,在实体肿瘤治疗中有巨大潜在辅助治疗价值.
