本文讨论了50例肾移植术后病人全血环孢素浓度对T淋巴亚群和移植肾功能状态的影响。用TDx法测定全血环孢素浓度并同时用间接荧光免疫法测外周血T淋巴亚群:CD_3~+,CD_4~+,CD_8~+。将T淋巴亚群资料按不同环孢素峰、谷浓度分组,用t检验进行统计学处理。结果表明:在环孢素峰浓度过高(>1500μg/l)与过低组(<600μg/l)之间,CD_8~+,CD_4~+/CD_8~+值有非常显著差异(p<0.001);环孢素谷浓度过高(>800μg/l),过低(<400μg/l)时所发生的移植肾功能恶化次数远多于环孢素谷浓度处于中间范围者。本文认为:环孢素血浓度对T淋巴亚群以及移植肾的功能有明显的影响;环孢素峰、谷浓度在其作用中起不同贡献。因此建议临床不仅测定环孢素谷值,还要测定其峰值,并将它们控制在适当的范围里,以保护移植肾,延长其存活时间,井减轻环孢素对机体的过度免疫抑制作用及毒性。
This article discusses the effect of cyclosporin concentration in 50 patients after renal transplantation on functional status of T-lymphoid subgroups and graft kidney. The concentration of cyclosporine in whole blood was determined by TDx method and T lymphocyte subsets in peripheral blood were detected by indirect fluorescence immunoassay: CD_3 ~ +, CD_4 ~ +, CD_8 ~ +. T lymphocyte subsets of different cyclosporine peak by trough concentration group, using t test for statistical analysis. The results showed that there was a very significant difference (p <0.001) between CD8 + and CD4 + / CD8 + values at the high concentration of cyclosporine (> 1500μg / l) ); Cyclosporine Valley concentration is too high (> 800μg / l), too low (<400μg / l) occurs when the frequency of renal graft deterioration than the concentration of ciclosporin in the middle range. This paper argues that: Cyclosporine blood concentration has a significant effect on T lymphocyte subsets and renal grafting function; cyclosporine peak, trough concentration in its role in different contributions. It is therefore recommended that clinical not only determine the value of cyclosporine, but also to determine the peak value, and they are controlled in the appropriate range to protect the graft, to extend its survival time, and reduce the excessive immunosuppression of ciclosporin on the body and toxicity.
