据报道,面部骨折后的失明发生率为0.67%~3%。作者对1970年1月至1992年8月间发生的5936例面部骨折的病例进行了回顾性研究。结果发现19例患者有19只眼视力丧失,大部分(12例)为道路交通事故所致。男性15例,女性4例,13例年龄在10~40岁。发生于单侧颧眶复合骨折8例,双侧1例,Le Fort Ⅲ型骨折8例,Le Fort Ⅱ型骨折2例,其中11例还有下颌骨骨折,6例有颅骨骨折。19例中,右眼10例,左眼9例。失明原因10例(52%)是因球后出血,5例(26%)因整个眼睛严重受伤,2例(10%)为视神经撕裂伤,1例(5%)是视神经受压,还有1例(5%)是术后明显出血和血肿形成所致。
It is reported that the incidence of blind after facial fracture was 0.67% to 3%. The authors retrospectively reviewed 5936 cases of facial fractures that occurred between January 1970 and August 1992. The results showed that 19 patients had 19 eyes loss of vision, the majority (12 cases) due to road traffic accidents. There were 15 males and 4 females, 13 of whom were 10 to 40 years old. Occurred in unilateral zygomatic orbital complex fracture in 8 cases, bilateral in 1 case, Le Fort Ⅲ fracture in 8 cases, Le Fort Ⅱ fracture in 2 cases, of which 11 cases also had mandibular fractures, skull fractures in 6 cases. In 19 cases, there were 10 cases in the right eye and 9 cases in the left eye. Ten patients (52%) of the causes of blindness were due to posterior capsular hemorrhage, five (26%) were severely injured by the entire eye, two (10%) were optic nerve lacerations and one (5%) were optic nerve compression One case (5%) was postoperative obvious hemorrhage and hematoma formation.
