目的探讨CD44V6蛋白在口腔鳞状细胞癌(Oral Squamous Cell Carcinoma,OSCC)发生发展过程中的作用及意义,为OSCC的临床诊断、治疗、预后以及生物学评估提供新的思路和理论依据。方法选择58例口腔鳞癌组织、15例口腔粘膜上皮异常增生组织和11例正常口腔黏膜组织,应用PV-9000两步免疫组织化学染色法检测CD44V6蛋白的表达情况。结果 CD44V6在正常黏膜、上皮异常增生口腔黏膜、鳞癌三组的阳性率分别为100%、93.33%、56.90%,三组总体差异及组间差异均有统计学意义(P<0.05)。在OSCC组,高、中、低分化三组的强阳性率分别为16.00%、9.52%、8.33%,三组间总体比较有显著性差异(P<0.05)。在OSCC组,患者按年龄、性别、临床分期、瘤体大小等分组间比较CD44V6表达无显著性差异(P>0.05);在按有无淋巴结转移以及分化程度组间比较CD44V6的表达差异有显著性差异(P<0.05)。结论 CD44V6在口腔鳞癌的发生、发展以及转移的过程中发挥着一定的作用。CD44V6在OS-CC组织中呈下行表达,与其性别、年龄、瘤体大小以及临床分期无关,但与OSCC的局部淋巴结转移和病理分化程度有关,可考虑作为一种有效的检测OSCC的标志物,并作为预测其预后的检测指标。
Objective To explore the role and significance of CD44V6 protein in the development and progression of oral squamous cell carcinoma (OSCC) and to provide new ideas and theoretical basis for the clinical diagnosis, treatment, prognosis and biological evaluation of OSCC. Methods 58 cases of oral squamous cell carcinoma, 15 cases of oral mucosal epithelial dysplasia and 11 cases of normal oral mucosa were selected. The expression of CD44V6 protein was detected by PV-9000 two-step immunohistochemical staining. Results The positive rates of CD44V6 in normal mucosa, epithelial dysplasia, oral mucosa and squamous cell carcinoma were 100%, 93.33% and 56.90% respectively. There were significant differences among the three groups (P <0.05). In the OSCC group, the strong positive rates of high, moderate and poorly differentiated groups were 16.00%, 9.52% and 8.33%, respectively, with significant differences among the three groups (P <0.05). In the OSCC group, there was no significant difference in the expression of CD44V6 between groups according to age, sex, clinical stage and tumor size (P> 0.05). There was significant difference in the expression of CD44V6 between groups with or without lymph node metastasis and differentiation Sex differences (P <0.05). Conclusion CD44V6 plays a role in the carcinogenesis, development and metastasis of oral squamous cell carcinoma. CD44V6 is down-regulated in OS-CC tissues, regardless of gender, age, tumor size and clinical stage, but related to the degree of local lymph node metastasis and pathological differentiation of OSCC. CD44V6 can be considered as an effective marker for detecting OSCC, And as a prognostic indicator of the prognosis.
