目的探讨臭氧液阴道冲洗治疗对于宫颈人乳头瘤病毒(HPV)单一亚型感染的清除效果。方法采用随机对照研究方法,在4家医院将263名HPV单一亚型感染病除外宫颈上皮内瘤样病变二级(CIN2)及以上的研究对象经知情同意后随机分为3组,分别是臭氧连续阴道冲洗一周组(A组,92例)、臭氧间断阴道冲洗4天组(B组,87例)以及空白对照组(C组,84例),并在治疗后第3个月和第6个月进行随访,阳性的HPV亚型转阴者为有效,未转阴者视为无效。记录并比较各组患者的处理效果。结果 (1)研究对象于阴道冲洗3个月后首次随访,A、B、C组有效的比例分别为66.3%(61/92)、57.5%(50/87)和51.2%(43/84),三组间无统计学差异(χ2=4.196,P=0.123);(2)研究对象于6个月后第2次随访,三组间有效的比例分别为58.6%(17/29)、41.2%(14/34)和20.5%(8/39),其中A组有效率显著高于C组(χ2=10.415,P=0.005);(3)三组研究对象随访两次的总有效率分别为85.9%(79/92)、74.7%(65/87)和61.9%(52/84)。A组与C组相比,疗效增加了38.8%(χ2=13.251,P<0.001);B组与C组相比,疗效增加了20.7%,但无统计学差异(χ2=3.245,P>0.05);A组与B组比较,疗效增加了15.0%,但无统计学差异(χ2=3.538,P>0.05)。结论虽然HPV感染大多可自然清除,但对于反复HPV感染者或HPV持续感染并有焦虑情绪的患者,应用臭氧液阴道连续冲洗一周可作为加速HPV病毒清除的一种方法。
Objective To explore the effect of ozone liquid vaginal irrigation on the removal of cervical single-subtype infection of human papillomavirus (HPV). Methods A randomized controlled study was conducted in four hospitals except 263 cases of single subtype of HPV infection with cervical intraepithelial neoplasia grade 2 (CIN2) and above subjects were randomly divided into three groups with informed consent: ozone The rats in group A were treated with continuous vaginal washing for one week (group A, n = 92), intermittent vaginal washing with ozone for four days (group B, 87), and blank control group (group C, n = 84) Month follow-up, positive HPV subtypes were negative, non-negative as invalid. Record and compare the treatment effect of each group of patients. Results (1) The subjects were followed up for 3 months after vaginal irrigation. (2) The subjects were followed up for 6 months and the effective rates were 58.6% (17/29) and 41.2 (14/34) and 20.5% (8/39) respectively. The effective rate of group A was significantly higher than that of group C (χ2 = 10.415, P = 0.005). (3) The total effective rates of two groups were 85.9% (79/92), 74.7% (65/87) and 61.9% (52/84). Compared with group C, the curative effect of group A increased 38.8% (χ2 = 13.251, P <0.001); the curative effect of group B increased 20.7% compared with group C, but there was no statistical difference (χ2 = 3.245, P> 0.05 ); A group and B group, the effect increased by 15.0%, but no significant difference (χ2 = 3.538, P> 0.05). Conclusions Although most HPV infections are naturally cleared, the continuous flushing of the vaginal fluid with ozone fluid for one week may be used as a method of accelerating the clearance of HPV in patients with recurrent HPV infection or persistent HPV infection and anxiety.
