目的:探讨彩超监测肠系膜上动脉流速对粘连性肠梗阻的诊断及药物疗效的判定价值。方法:72只SPF级(雌雄各半)Wistar大鼠,随机分为正常组、模型组、大承气汤(DS)组、茴香枳术汤(HX-S)低剂量组、中剂量组和高剂量组,除正常组,其余组大鼠按照浆膜剥脱法复制大鼠粘连性肠梗阻模型,造模成功后各组分别检测肠系膜上动脉流速及肠梗阻程度,并对其关系进行统计学分析;各组分别给予生理盐水、DS、HX-S低、中、高剂量灌胃,治疗后第3、7天分别应用彩色多普勒观察大鼠肠系膜上动脉峰值流速。结果:模型组中肠系膜上动脉(superior mesenteric artery,SMA)收缩期峰值流速与肠梗阻严重程度呈正相关(r=0.418,P<0.01);治疗后第3、7天,DS组、HX-S低、中、高剂量组与模型组比较,肠系膜上动脉收缩期峰值流速均明显下降(P<0.05),各治疗组比较,HX-S高剂量组效果较显著。结论:应用高频彩超监测肠系膜上动脉流速对大鼠粘连性肠梗阻的诊断及药物疗效的判定有一定的价值。
Objective: To explore the value of monitoring the superior mesenteric artery flow velocity in the diagnosis of adhesive intestinal obstruction and the therapeutic value of the drug. Methods: Seventy-two Wistar rats were divided into normal group, model group, Dachengqi Decoction (DS) group, HX-S low dose group, middle dose group and High dose group, in addition to the normal group, the rest of the rats in accordance with the method of serosal exfoliation replication model of adhesive intestinal obstruction, the success of each model were detected after the superior mesenteric artery flow velocity and intestinal obstruction, and the relationship between the statistics The rats in each group were given physiological saline, DS, HX-S low, medium and high dose gavage respectively. On the 3rd and 7th day after treatment, the peak velocity of the superior mesenteric artery was observed by color Doppler. Results: The systolic peak flow velocity of the superior mesenteric artery (SMA) in model group was positively correlated with the severity of intestinal obstruction (r = 0.418, P <0.01). On day 3 and 7, Compared with the model group, the peak velocity of systolic peak of the superior mesenteric artery in the low, medium and high dose groups was significantly decreased (P <0.05). Compared with the model group, the HX-S high dose group had more significant effect. Conclusion: It is of value to use high frequency color Doppler to monitor the velocity of superior mesenteric artery in the diagnosis of adhesive intestinal obstruction and the determination of therapeutic effect.
