目的探讨杂交捕获二代(HC2)人乳头瘤病毒(HPV)DNA检测与宫颈液基细胞学涂片对于宫颈病变的诊断价值。方法 2012年1-12月选取潍坊市人民医院妇科门诊行杂交捕获二代人乳头瘤病毒DNA检测患者1057例,其中同时进行宫颈液基细胞学涂片筛查604例,比较二者对于宫颈病变的诊断效能。结果 HPV感染率和病毒载量在临床分组NILM组、低度病变(ASCUS/LSIL)组、高度病变(HSIL/SCC)组之间存在显著性差异(P<0.05)。经Kappa一致性检验,人乳头瘤病毒检测与细胞学检测相关系数为0.26,无明显相关性。结论 HPV感染率和病毒载量与宫颈细胞学分级相关。LCT与HC2两种检测方法不具有关联性,因此不能互相取代,如果联合两种检测方法,几乎可以筛查出所有的CIN2级以上病变,明显提高宫颈癌病变的检出率。
Objective To investigate the diagnostic value of hybridization capture second generation (HC2) human papillomavirus (HPV) DNA and cervical liquid - based cytology smears for cervical lesions. Methods From January to December in 2012, Weifang People’s Hospital gynecology outpatient clinically selected 1057 cases of second-generation human papillomavirus DNA test, of which 604 cases of cervical liquid-based cytology smear screening at the same time, comparing the two for cervical lesions The diagnostic efficacy. Results HPV infection rate and viral load were significantly different between the NILM group, the low grade lesion group (ASCUS / LSIL) and the high grade lesion group (HSIL / SCC group) (P <0.05). Conclusion HPV infection rate and viral load are related to cervical cytology. LCT and HC2 two detection methods are not associated with each other and therefore can not replace each other, if the combination of two detection methods, almost all of the CIN2 lesions can be screened, significantly improve the detection rate of cervical lesions.
