目的探讨负压封闭引流技术(VSD)结合自体植皮术治疗慢性创面的临床效果。方法本组28例慢性创面在基础病治疗的同时,采用VSD治疗1~3次,然后自体植皮术+VSD包扎治疗,观察疗效。结果 1例直接愈合,2例减张缝合治愈,其余25例均经自体植皮+VSD包扎治疗创面愈合。随访6~12个月,除1例电击伤者右膝关节功能轻度障碍外,其余均效果良好,外观平整无复发。结论 VSD结合自体植皮术治疗慢性创面,能充分引流,清洁创面,控制感染,促进肉芽组织生长和创面愈合。同时可降低手术风险,缩短疗程,减轻患者痛苦,减轻医务人员的工作压力,是一种简单、安全有效值得推广的治疗方法。
Objective To investigate the clinical effect of negative pressure closed drainage (VSD) combined with autologous skin grafting in the treatment of chronic wounds. Methods The 28 cases of chronic wounds treated with VSD were treated with VSD 1 ~ 3 times, and then autologous skin grafting + VSD bandaging treatment to observe the curative effect. Results One case healed directly and the other two cases were cured by suture tension reduction. The other 25 cases were treated with autologous skin grafting and VSD bandaging for wound healing. All the patients were followed up for 6 ~ 12 months. Except for mild electric shock of right knee in 1 case, the rest were well and the appearance was flat without recurrence. Conclusion VSD combined with autologous skin grafting for the treatment of chronic wounds can fully drain and clean the wounds, control the infection and promote the growth of granulation tissue and wound healing. At the same time can reduce the risk of surgery, shorten the course of treatment, reduce patient suffering, reduce the pressure of medical staff, is a simple, safe and effective treatment worth promoting.
