五味子治疗维生素A缺乏型干眼症疗效及作用机制

目的:观察五味子治疗维生素A缺乏型干眼症的疗效及眼角膜组织中IL-6、TNF-α的表达变化,比较泪腺细胞凋亡的数量,探讨酸味中药缓解干眼症的作用机制。方法:将180只白色来亨鸡随机分为正常组、模型组(维生素A缺乏组)、维生素A对照组、五味子治疗组(高、中、低剂量组)。分别于用药前、用药后2周、用药后4周对各组来亨鸡进行Schirmer I试验(SIT)和泪膜破裂时间(BUT)检查。对角膜做荧光素染色并用免疫组织化学检测白介素-6(IL-6)和肿瘤坏死因子-α(TNF-α),并比较各组泪腺细胞凋亡的数量。结果:用药2周、4周后,与模型组比较,Schirmer试验滤纸湿长有明显改变,泪膜破裂时间逐渐增加,角膜表面湿润度增多,荧光素染色少量着色。免疫组织化学检测发现IL-6和TNF-α在各组来亨鸡角膜上皮细胞中表达显著降低,泪腺细胞凋亡明显缓解。结论:五味子可以提高泪液基础分泌量,维持泪膜稳定性,其机制可能是抑制泪腺细胞凋亡,抑制IL-6和TNF-α表达,减轻炎症反应。 OBJECTIVE: To observe the curative effect of Schisandra chinensis in the treatment of vitamin A-deficiency dry eye and the changes of the expression of IL-6 and TNF-α in the cornea of ​​the eye, and to compare the apoptosis of lacrimal gland. Methods: 180 white Leghorn chickens were randomly divided into normal group, model group (vitamin A deficiency group), vitamin A control group and Schisandra chinensis treatment group (high, medium and low dose group). The Schirmer I test (SIT) and tear film break-up time (BUT) were performed in each group before treatment, 2 weeks after treatment and 4 weeks after treatment. Fluorescein staining was performed on the cornea and the levels of interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) were detected by immunohistochemistry. The numbers of apoptosis of lacrimal gland in each group were also compared. Results: After 2 weeks and 4 weeks of treatment, compared with the model group, the wet filter paper of Schirmer test significantly changed, the tear film rupture time gradually increased, the corneal surface wettability increased, and fluorescein staining a small amount of staining. Immunohistochemistry showed that the expression of IL-6 and TNF-α in corneal epithelial cells of each group were significantly decreased and the apoptosis of lacrimal gland cells was relieved. Conclusion: Schisandra can improve the basic secretion of tear fluid and maintain the tear film stability. Its mechanism may be to inhibit the apoptosis of lacrimal gland cells, inhibit the expression of IL-6 and TNF-α, and reduce the inflammatory reaction.
