洋地黄治疗心力衰竭已有200年的历史,至今仍为主要药物,过去因袭已久的心力衰竭及洋地黄作用的概念,现时有了不少新的认识。一、洋地黄的血液动力学作用洋地黄的最主要和最基本的作用是心肌的正性收缩能力,现时对其血管收缩作用已有重视,洋地黄的血液动力学作用,归纳为心肌正性收缩能力和血管收缩作用的综和。心肌功能状态,最好以Frank-Starling心室功能曲线表示:(图1)
