患儿女,8天。因腹胀、发烧8天,面色苍白,黑便3天于1986年9月2日入院。第3胎足月产,第二产程延长3小时。生后青紫窒息1小时,复苏后无哭声。4小时后突然出现腹胀,发热、体温达40℃。当地按败血症治疗,热退但腹胀更明显。入院前3天出现面色苍白,大便呈柏油样稀便,每日2～3次,呼呼急促,拒乳,反应差,入院当日全身皮肤出现大小不等的瘀斑,母孕期健康,无用药史,家族中无传染病及遗传病史。体检:体温35.5℃,脉搏160次,呼吸60次,体重2700g。面色极度苍白,呼吸浅表不规则周身皮肤可见散在大小不等瘀斑,无黄染,前囟3.5×3.5cm,饱满,张力高,双侧瞳孔散大等圆,对光反应迟钝,两肺无异常,心音低钝,腹部高度膨隆,胀气,腹壁静脉显露,无肠型及蠕动波,肝脾未触及,肠鸣音减弱,四肢肌张力弱,拥抱反射及吸吮反时消失。血红蛋白3.2g,白细胞7600,中性粒细胞72％,淋巴细胞28％,血小板3000,DIC筛选试验:PT15秒(对照14秒),TT13秒,(对照13秒),3P试验阳性。血气:pH7.192PQ_245.9mmHg,PCO_259.2mmHg,BE
