目的探讨超声造影在乳腺癌患者中的诊断效果。方法分析2014年2月—2017年2月信阳市中心医院收治的116例乳腺肿块患者资料,按病理结果分为乳腺癌组(84例)、良性乳腺病灶组(32例),均行超声造影,记录诊断乳腺癌准确率、灵敏度、特异度,观察造影剂状态及病灶形态,记录乳腺癌与良性乳腺病灶、各类型乳腺癌间时间-强度曲线定量参数。结果诊断乳腺癌准确率、灵敏度、特异度分别为86.98%、91.28%、72.68%;造影剂在癌灶中主要为快进快出、不规则边界型,在良性病灶中主要为无造影剂进入或缓慢退出和进入;乳腺癌病灶主要表现为边界不清晰、形态不规则、多有扭曲或穿入血管;良性病灶为边界清晰、形态规则、极少数有扭曲或穿入血管。乳腺癌TTP较良性乳腺病灶低,PI较良性乳腺病灶高,差异均有统计学意义(P<0.05);在不同类型乳腺癌中部分组间存在显著TTP或(和)PI差异,对比有统计学意义(P<0.05)。结论超声造影能通过造影剂状态、病灶形态、时间-强度曲线诊断乳腺癌;时间-强度曲线有利于乳腺癌分型判断。
Objective To investigate the diagnostic value of contrast-enhanced ultrasound in breast cancer patients. Methods The data of 116 cases of breast lumps treated in Xinyang Central Hospital from February 2014 to February 2017 were divided into breast cancer group (84 cases) and benign breast group (32 cases) according to the pathological findings. All patients underwent ultrasound contrast The diagnostic accuracy, sensitivity and specificity of breast cancer were recorded. The quantitative parameters of the time-intensity curve between breast cancer and benign breast lesions and various types of breast cancer were recorded. Results The accuracy, sensitivity and specificity of the diagnosis of breast cancer were 86.98%, 91.28% and 72.68% respectively. The contrast agent was mainly fast-moving fast and irregular in the foci, Or slowly exit and enter; breast lesions mainly for the border is not clear, irregular shape, many have distorted or penetrated into the blood vessels; benign lesions as a clear boundary, the rules of morphology, a very small number of distorted or penetrating blood vessels. The TTP of breast cancer was lower than that of benign breast and PI was higher than that of benign breast (P <0.05). There were significant differences of TTP or (and) PI between some groups in different types of breast cancer Significance (P <0.05). Conclusions Ultrasound can diagnose breast cancer by contrast agent state, lesion morphology and time-intensity curve. The time-intensity curve is helpful to the classification of breast cancer.
