目的调查北京市顺义区2010-2014年人间布鲁氏菌病(布病)的感染情况,了解流行病学特征,为制定防控策略提供科学依据。方法对布病重点职业人群的血清标本进行虎红平板凝集试验检测,阳性或可疑标本进一步利用试管凝集试验判定。对高危职业人群布病感染情况及人间发病情况进行描述性分析。结果 2010-2014年顺义区共报告病例35例,呈逐年上升趋势(χ~2=15.002,P<0.05);农民发病病例占51.43%,71.43%的患者发病前有牛、羊接触史。累计检测高危人群1 025人,阳性20人,阳性率为1.95%,重点职业人群布病抗体阳性率呈逐年下降趋势(χ~2=16.960,P<0.05),其中直接接触牛、羊的职业人群布病抗体阳性率分别为4.37%和0.54%,差异有统计学意义(χ~2=6.031,P<0.05);农村地区、城区及城乡结合部职业人群阳性率分别为3.21%和0.48%,两地区布病抗体阳性率差异有统计学意义(χ~2=8.807,P<0.05)。结论顺义区人间布病发病呈上升趋势,直接接触牛的人群为高危职业人群,应加强对顺义区重点职业人群布病的监测和健康宣传教育。
Objective To investigate the prevalence of brucellosis (brucellosis) in Shunyi District of Beijing from 2010 to 2014, understand the epidemiological characteristics and provide a scientific basis for the development of prevention and control strategies. Methods Serum samples of brucellosis occupational groups were assayed by tiger red plate agglutination test. Positive or suspicious samples were further determined by tube agglutination test. Descriptive analysis of brucellosis and human morbidity in high-risk occupational groups. Results A total of 35 cases were reported in Shunyi District from 2010 to 2014, showing an upward trend year by year (χ ~ 2 = 15.002, P <0.05). The incidence of peasants accounted for 51.43% and 71.43% before the onset of disease. A total of 1 025 high-risk groups were tested, with a positive rate of 1.95%. The positive rate of brucellosis antibody in key occupational groups showed a declining trend year by year (χ ~ 2 = 16.960, P <0.05), of which the direct contact with cattle and sheep The prevalence of brucellosis was 4.37% and 0.54% respectively in the population (χ ~ 2 = 6.031, P <0.05). The positive rates of occupational population in rural areas, urban areas and urban-rural areas were 3.21% and 0.48% The positive rates of brucellosis antibody in the two regions were significantly different (χ ~ 2 = 8.807, P <0.05). Conclusion The prevalence of brucellosis in Shunyi District is on the rise. People who directly contact with cattle are high-risk occupational groups. Monitoring and health promotion and monitoring of brucellosis among key occupational groups in Shunyi District should be strengthened.
